Disease Management
Our Disease Management program assists if you have chronic illnesses such as asthma, coronary artery disease (CAD), congestive heart failure (CHF), diabetes, depression, and alcohol/substance abuse. The goal of the program is to help you improve or maintain your health while living with a chronic illness. Disease Management services include telephone contact and assessment of your current health status, educational information about the disease and treatment, and the coordination of services through close contact with your physician.
The disease management program offers one-on-one counseling and/or disease education. At the heart of this patient-focused, personalized approach is a team of experience nurses, social workers, and an alcohol and substance abuse counselor, whose ongoing support:
- empowers you to take a more active role in your own disease management therapy through the HIRSP education programs and communication materials;
- improves compliance with prescribed therapies; and
- enhances both clinical and medical resource outcomes.
HIRSP also offers a prenatal program with maternity nurse specialists assisting policyholders throughout their pregnancies by providing educational opportunities and coordination of services.
