Stay in Control of Your Diabetes with Help from LifeMasters

HIRSP is now offering a diabetes management program through LifeMasters that will help you keep your diabetes under control. You'll receive support from your own personal health professional who will talk with you over the phone to help you manage your condition. You'll also receive help checking and tracking your blood sugar, managing your diet and scheduling the tests you need. Diabetes with Help from LifeMasters

When it comes to managing diabetes, a little support can go a long way. HIRSP is now offering a health improvement program called LifeMasters that will help you keep your diabetes under control. It is only available to members with diabetes at no additional cost.

Experienced LifeMasters nurses and health coaches will talk with you over the telephone to support your plan of care.  You’ll receive help checking and tracking your blood sugar, managing your diet and scheduling the tests you need.

The goal of the LifeMasters program is to help you improve your health, avoid complications, reduce the need for some medications and stay out of the hospital. It is also completely confidential.

In between visits with your doctor, LifeMasters nurses will notify your doctor of any problems you may have with controlling your diabetes. If you participate, you will receive:

  • Information about your health conditions
  • Education and tools to help you keep track of your health and progress
  • Help in setting realistic health goals
  • Help understanding and following your doctor’s advice
  • Equipment such as blood pressure monitors or scales if you need it
  • Information about exercise, healthy meal choices and food substitutions, customized for you

Those who are diabetic and eligible for the program should have received a telephone call welcoming you to the LifeMasters program.
