That's why we've partnered with StayWell® Health Management to offer eligible participants the opportunity to participate in a diabetes management program. Through this program, StayWell health coaches will work with you to develop action plans based on your specific health needs and goals. We'll help you adopt and maintain healthy behaviors by creating a plan that considers your own personal needs, strategies for improving your health, and your support system. Or you may call a health coach at 1-888-801-8876 to begin your diabetes management program on your own terms. Participation is over the phone and is completely voluntary. Participation or non-participation in this program will in no way impact your HIRSP benefits. With a little knowledge and the will to make small, realistic changes, nothing stands in your way!
